In 2023, we introduced the Meaningful Life Model of Care—a transformative approach that places residents at the heart of everything we do, focusing on personal choice, lifestyle preferences, and community connection. Through partnerships with families, volunteers, and local organisations, we create enriching experiences that make each day purposeful.
To support this model, we’ve appointed 14 Meaningful Life Coordinators and a network of Meaningful Life Partners across our Care Communities, ensuring every resident receives holistic support and meaningful engagement.
Our communities feature height-adjustable electric beds, nurse call-bells in key areas, and varied room layouts tailored to individual needs. Personal laundry is handled in-house, while linen and towels are professionally cleaned off-site for added comfort and convenience.
Whether you're looking for quiet relaxation, vibrant social interaction, or specialised clinical care, Grampians Health Aged Care Communities provide a warm and welcoming place to call home.
